Cruise Holidays will apply the cruise deposit, final payment, travel insurance premium, pre- and/or post-cruise tours and any other cruise related charges to individual group member credit or debit cards. Each charge will be confirmed with the individual before the charge is made and no charge will be incurred without prior authorization. The details about each group member card or cards must be on file in our office along with the member permission to use the card(s). Please complete the Credit Card Authorization form and return to our office by US Mail, fax, or email as outlined below:
