In order to begin cruise or cruise tour registration of individual members in a group, there is personal information about each person traveling including the group leader that must be collected before each individual booking can be completed and a reservation confirmed. This information is required by Cruise Holidays, the cruise lines and/or the US Government. Please complete our Personal Information Form (PIF), one per person for each member of your party, and return to our office by US Mail, fax or email as outlined below:
